入所（面接）にはどういったものが必要ですか？
履歴書（写真付きのもの）1通が必要です。
入所の意思決定の後、障がい者福祉サービス受給者証の申請をして頂き、発行されれば正式に入所となります。
※自分の障がいの程度や、サービス受給者証がないという方も、まずはご質問下さい。
