利用するために障害者手帳は必要ですか？
障害者手帳をお持ちでない方も、対象疾患の方であれば利用できます。
ただし、「障害福祉サービス受給者証の申請」の際、障害者手帳をお持ちでない方は医師の意見書等が必要になります。詳しくは、お住まいの市区町村の役所の窓口にお問合せください。
